These are only the Posters offered in the Goal Setting & Data Portfolios. They are not grade level specific and do not have standards listed on them.
Start setting academic and behavior goals with your students. Through classroom discussions about goal setting students are introduced to the concept of SMART goals and learn how to set a realistic goals.
Through individual conferencing, students use classroom data to set goals, set an action plan, and reflect on their learning. Students set goals based on their own individual data.
Each of the forms has grade level common core standards and a place to write assessment data that could be used to assess that standard. Student then list activities that help them meet their goal and decide on one or two activities on which to focus.
