This packet will help 5th graders to start setting academic and behavior goals with goal setting forms, checklists, data tracking forms, and more.
Help students start setting academic and behavior goals. Through classroom discussions, students are introduced to the concept of SMART goals and learn how to set a realistic goal.
Through individual conferencing, students use classroom data to set goals, set an action plan, and reflect on their learning. Students set goals based on their own individual data.
Each of the goal-setting forms has fifth-grade common core standards and a place to write assessment data that could be used to assess that standard. Student then list activities that help them meet their goal and decide on one or two activities on which to focus. Also included is a matrix for each standard so that students can record assessment data or scores.
